How to Enroll
By Mail or Fax: Download and complete the enrollment form , listing the individual(s) and roundtable(s) they will be joining. Mail the form with your check (made payable to the Manufacturer & Business Association) or credit card information, or fax it to 814/833-4844.
Once we receive payment, you will be notified by fax of all meeting dates. If you have questions on enrollment or would prefer to have an enrollment form faxed to you, please contact Laurie Mattis at 814/833-3200 or 800/815-2660.
